Everything you need to know to play a sport at Penndale:
All athletes will be required to register online each season. Visit the Google link for directions: Online Registration
It is necessary to have a PIAA Comprehensive physical evaluation signed and dated by an authorized medical examiner. (Please note: You will also be able to print all forms from the FormReleaf Online Registration)
PIAA Physical Form Section 5 - Health Form, Section 5
PIAA Physical Form Section 6 - Physician Form, Section 6
Hard copies of pages 5 & 6 are available in the main office. Please see Mrs. McManus.
-
We cannot accept other physical forms even if they look identical. It must be the PIAA form with Section 5 filled out by a parent/guardian and section 6 completed, signed and dated by a physician.
-
The physical must be performed after May 1ST of the current school year which will make it valid until May 1st of the following year. The physical can not expire during a sports season.
-
It is strongly recommended that you schedule an appointment soon, as appointments for physicals fill up quickly. Patient First and Walgreens provide sports physicals at a reasonable cost for those whose insurance will not cover one during the required time frame.
-
NO STUDENT (UNDER ANY CIRCUMSTANCES) WILL BE PERMITTED TO PARTICIPATE IN A TRY OUT OR PRACTICE UNTIL THEY COMPLETE THE ONLINE REGISTRATION FORM AND SUBMIT THE COMPLETED PIAA PHYSICAL EVALUATION FORM SECTIONS 5& 6. Completed forms should be upload to FormReleaf.
LATE BUS
North Penn Offers a Late Bus for students to take home on Tuesdays and Thursdays at 5:00. Students board the bus and give the driver their address and they are taken home. Please note that this is only offered to students who regularly ride a bus. Students who walk home from school may not take the Late Bus.
Sports Shuttle
9th grade students are able to tryout/play sports at North Penn High School. For those students who make the team, there is a daily Sports Shuttle that takes them to the high school at the end of the day for practices and games. The Sports Shuttle is available at dismissal in the Bus lane outside the main entrance of the school.
